Open the original image:
Duplicate the layer and apply the Gaussian Blur filter:
Change the layer mode to Multiply, Ctrl+E to flatten the layers, and then adjust the curves:

Merge all the text layers, then duplicate the text layer, apply the Gaussian blur filter and adjust the layer mode and opacity:
Now to create the smoke effect, find a picture of smoke, then select it and add it to the current layer to create a new layer:
ctrl+I to invert, then Ctrl+T for free transformation:
Then use the brush tool with the following settings:
Paint some parts:
