Now data mining has revealed that there should indeed be something to do with cows, and there are three mysterious items that may be related to it.
The first is a bloody wood splinter (probably White's prosthetic leg), which data mining shows dropped in Hawize or Khakistan.
The second one is an old scroll (the second-generation teleportation book), which drops in Thorgren and Broken Peaks.
The third one is a metal fragment (corresponding to the Horadrim block), dropped in dry plains.
After collecting these three, go to the Cow God Fountain and you should be able to open the Cow Level.
It is also shown that this mysterious task is automatically reset every 24 hours, and these items are not likely to be dropped by killing monsters, but like being dropped from completing events, and the drop probability is 5%.
